Online Form Requests with Fees is the third module that builds on our “multi-page form” engine (read more under Free Features below) and allows you to create customized online forms that can be completed via the portal.
For example, you may create a back-to-school form to collect up-to-date health and emergency contact information and assign it to all children in grades 3-8. All guardians of those children will be alerted via email and the form will show up in their portal. The form will pull existing information from the child’s record, such as allergies, medications, insurance information, etc. The parent can confirm each of these and make changes as needed, including uploading any supplementary documentation you require. Keep track of which parents have submitted, which have begun but not submitted and which have yet to begin the completion of these forms.
